New Patient Paperwork

New patient paperwork (electronic) -  Click the red hyperlink to the left to pre-register if you do not yet have a new patient appointment.  You can fill out your demographics here and the new patient intake questionaire to answer the necessary health questions so we can better serve you.  This will help you see the doctor as quickly as possible on your visit.  We will call you for an approintment to schedule after we receive your pre-registration information.

New patient paperwork (for printing) - Please click on the red lettering hyperlink to the left to print your new patient paperwork and fill out in advance of your appointment.  Please bring this with you to your appointment (or bring in prior to your appointment).  Please arrive 30 minutes prior to your scheduled appointment for entry of this paperwork and regsitration.

American Thyroid Association - Excellent resource for information on thyroid disease.  It is evidence-based.

Medtronic Diabetes - Check status of orders, replace failed sensors, and order supplies directly from Medtronic.

Patient Assistance Websites and Savings Cards

Novo Nordisk - A site to get savings cards for Novo products, patient assistance, and access to $99 insulin (even without insurance coverage).  Novo makes: Tresiba, Levemir, Novolog, Fiasp, Novolog 70/30mix, Novolin 70/30 mix, N, R, Victoza, Ozempic, Rybelsus, and Xultophy 100/3.6.

Sanofi - Patient assistance applications and savings cards including a card for $99 insulins for cash cost without insurance.  Sanofi makes: Lantus, Toujeo, Toujeo Max, Apidra, Admelog, and Soliqua 100/33.

Lilly - Patient assistance information and phone numbers from the makers of Humalog, Basaglar, Humalog 75/25 mix, Humulin R, N, and 70/30 mix, Humulin R U500, Trulicity.  They are currently also offering $35 insulin copays for uninsured and commercial insurances if you call (833) 808-1234 for assistance during COVID-19.

AstraZenica - Patient assistance information and applications.  Savings cards for medications.  AstraZenica makes: Bydureon, Onglyza, Kombiglyz XR, Farxiga, Xigduo XR, Qtern, Symlin, and Byetta.

Boehringer Ingelheim - Savings card for Jardiance direct link.  The company makes Jardiance, Synjardy XR, Glyxambi and offers patient assistance as well from their site.

Merck - Patient assistance program for their products.  They make Januvia, Janumet, Steglatro, Steglujan.  Please search individually for the products on the web for savings cards for low to no-cost medications on commercial insurance.

GoodRx - A great place to find the best prices on medications in your local area and reduce cost with free coupons. Also available with a downloadable app, available for Android and iPhone. - Fill out patient assistance paperwork from multiple different pharmaceutical companies to get help to cover your medications.

Social Security - For Medicare patients, you may eligible to receive your medications at very low cost through the Social Security Administration’s Extra Help program. Click the link to find out if you qualify, and submit your application online.
